OFFICIAL NHD T-SHIRTS
Please complete and return to me the t-shirt form.  I need a valid mailing address and t-shirt size.  One t-shirt per student attending nationals is provided.  If you would like an additional shirt they are $15 each.  Forms are due by May 14th--no exceptions

NHD NATIONAL CONTEST SCHOLARSHIP INFORMATION
If you wish to apply for financial assistance to attend the NHD National Contest in College Park, MD please read the requirements for filing for assitance and fill out the application form and return no later than May 15th!

All Registrants will need the following items to register for the Kenneth E. Behring National History Day Contest:
•    Home Telephone Number
•    Home Contact Information
•    Your Email Address (Valid email addresses are required.)
•    Purchase Order Number (if applicable)

Students will need these additional items:
•    Teacher Name and School
•    Chaperone Name
•    Entry Category, Division and Title
•    Names of All the Students in a Group Entry
•    Description of Entry
•    Medical and Health Information
•    Medical Insurance Policy Number
•    Purchase Order Number (if applicable)
•    Contact Information for Local Television Stations & Newspapers (optional)

National History Day is an educational program for students in grades 4-12.* Students research and develop topics related to an annual theme and present their conclusions in exhibits, performances, documentaries, websites or historical papers.

* National History Day is divided into three divisions:
