Application Instructions (for paper submission)

    • Please TYPE or PRINT legibly with pen. This information, including misspellings, will be used for programs, schedules and certificates.
    • Fill in ALL appropriate sections.
      Remember to provide all signatures — students, teacher, parent.
    • This form may be photocopied.
    • Read entry rules carefully!

All Categories:  Extension cords are not available

Documentaries:  DVD/VCR/TV will be available
                          Screen will be available

All other materials and equipment must be furnished by the student.
